Notary Service Checklist

Please make sure to bring the required document(s):

1. DOCUMENT/FORM TO BE NOTARIZED

Complete, but do NOT sign the document(s) ahead of time.

Please address all questions or concerns with the requesting or receiving agency of your document/form.

2. A VALID & CURRENT PHOTO ID 

Identification has to be current or has been issued within five years:

  • A Passport

  • California Driver's License

  • California DMV ID

  • Out-of-State Picture Driver's License

  • Alien Registration/Green Card Picture ID

  • Military ID

Driver's License is the best!

What if my ID has expired?

According to California law, if a signer doesn’t have a valid form of identification, it will be necessary for that person’s identity to be established by the oaths of two credible witnesses.

 

Before the oaths are administered, credible witnesses must present valid forms of identification as required by law, such as a driver’s license issued by the California Department of Motor Vehicles or a U.S. passport.

Under oath, the credible witness must swear or affirm that each of the following statements is true:

  1. The individual appearing before the notary public as the signer of the document is the person named in the document.

  2. The credible witness personally knows the signer.

  3. The credible witness reasonably believes the circumstances of the signer are such that it would be very difficult or impossible for the signer to obtain another form of identification.

  4. The signer does not possess any of the identification documents authorized by law to establish the signer’s identity.

  5. The credible witness does not have a financial interest and is not named in the document signed.

 

A credible witness must sign the notary journal and present a valid form of identification.
